• Pay: Starting at: $15.00 / hour

• This position provides transportation for patients accessing and utilizing services provided by Mosaic Life Care and attending physicians and affiliates. This position requires excellent verbal, written, and interpersonal skills.
• This position works under the supervision of the Manager of the Enterprise Contact Center and is employed by Mosaic Health System.

• Provides door-to-door services in the transportation of patients by assisting the patient in to the vehicle and to appropriate destination.
• Handles all incoming requests for transportation from designated personnel.
• Keeps van clean, serviced, and properly maintained.
• Responsible for maintaining a driving log consisting of date, time, destination, names of passengers, and odometer reading at departure and return.

• High School diploma or GED required.
• Chauffeur's license required upon hire.
• BLS certification required within 180 days of hire. EMT Certification preferred.
• Previous experience as a driver in transportation services required.
